If you have conditions such as diabetes, arthritis, high blood pressure, heart disease, chronic pain or anxiety, Living Well with Chronic Conditions can help you take charge of your life.
The workshop is conducted over four sessions by trained leaders with health conditions themselves, meeting weekly for two (2) hours per session. The overall goal is to enable participants to build self-confidence to assume an active role in maintaining their health and managing their chronic health conditions.
